Information on Hospital Stay

Admission Procedure

Elective Admission

  • We will contact you by phone in the morning on the day of admission.
    ※It is usually admit to hospital 13:00 to 14:00 in the afternoon, however, you may be instructed to admit in the morning by taking examination or physician’s order.
  • Please come to the Inpatient Flow Management Center on the 1st floor to check in.
  • We would like you to come to the hospital with anyone such as family or friends on admission.
  • Please make sure you have filled in all required forms such as "Application Form for Admission, Application Form for Guarantee Confirmation, Protection of Personal Information Agreement and Application Form for Hospital Clothes" to submit and bring it to the hospital.
  • Please show your insurance card and hospital ID card to our staff.
  • After completed admission procedure, the staff will guide you to the ward.

Other Admission

  • Please make sure you have filled in all required forms for your admission such as "Application Form for Admission, Application Form for Guarantee Confirmation, Protection of Personal Information Agreement and Application Form for Hospital Clothes" to submit to the Inpatient Flow Management Center.
  • Please show your insurance card and hospital ID card to our staff.
  • After completed admission procedure, the staff will guide you to the ward.

What to Bring on Admission

  • Toiletries (toothbrush set, shampoo, body soap, three towels and two bath towels)
  • Indoor slippers ※We recommend non-slip slippers
  • Tissue paper
  • Two to three bags to put underwear and laundry
  • If you are using diaper, you need to be used diaper provided by the hospital. (We will charge only used diapers.)
    ※If you wish to bring with you, please notify the nurse.
  • Please keep your possessions to a minimum.
  • If you have any currently-taking medication, please bring it to the hospital to check.

Rates are per-day and including tax.

Hospital Clothes & Bed Clothes

  • Hospital clothes are available to use for 80 yen per day.
    ※When you need to change hospital clothes, please let nurses know at any time.
  • If you do not wish to rend hospital clothes, please let nurses know and bring your own sleep clothes.
  • Bed clothes are available for no charge. We change bed sheet once a week.
  • Please refrain from bringing your own pillow or cushion.
    ※Please talk to the nurse, if you wish to bring those.

Hospital Meal

  • Hospital meal is cooked according to menu created by nutritionist considering proper quality and amount based on physician’s instruction as therapeutic diet.
  • Please notify us in advance, if you have any ingredients can cause an allergy or fall sick.
  • Please refrain from bringing food from outside hospital. Please talk to your primary physician or nurses if you wish.
  • Meal time
    Breakfast 8:00 Lunch 12:00 Dinner 18:00
    ※Meal time may change sometimes.
    ※Please note that depending on the time such as hospitalization or furlough due to the balance with closed accepting meal order, the start and stop of your meal may be from the next meal.
  • There is a fixed self-pay burden on meals during hospitalization expenses in principle.
  • Chopsticks and spoons are provided by hospital.

Bath

  • You absolutely need permission from your primary physician for your bathing.
  • Schedule for bathing is set for a period of time between 9:00 to 18:00.
  • When bathing, please let nurses know.
  • Treatment or assistance needed patients are given preference for bathing during the morning.
    ※Only patient can use shower room.

Lighting & Lights-out Time

  • Lighting time: 6:00 AM
  • Lights-out time: 22:00 PM(from May to October), 21:00(from November to April)

Visitation

In an effort to provide adequate rest for our patients, please follow our visiting hours.

  • In order to provide adequate rest and recuperation life to patients during hospitalization, visiting hours are set as below. Thank you for your cooperation.
  • Visiting hours for general ward: 15:00~20:00 (No allowed to visit after 20:00)
  • Visiting hours for ICU (No allowed to visit after 20:00)
    ⓵7:10~7:40 ⓶12:30~13:00 ⓷14:00~14:30 ⓸18:00~18:30 ⓹19:00~20:00
  • Visiting hours for HCU (High care unit) (No allowed to visit after 20:00)
    ⓵7:00~8:00 ⓶12:00~13:00 ⓷14:00~15:00 ⓸18:00~20:00
  • Visiting with children (especially infant) is discouraged due to prevention of infection.

When you visit patient, you need request for visitation and must obtain a visitor’s pass to visit a patient.

  • Monday to Saturday ⇒ 1. Visiting Patients counter at 2nd Floor
  • Sunday and Holiday ⇒ Guard Station at 2nd Floor (at after-hour entrance)
  • Weekdays nighttime (After 20:00) ⇒ Guard Station at 2nd Floor (at after-hour entrance)
    ※When you leaving hospital, please return the visitor’s pass.

Attendant during Hospitalization

  • Please refrain from having attendant during hospitalization as a general rule, however, permission may be given in case of special circumstance such as any patient's condition changes.
  • If you wish to attend the patient, you must obtain primary physician’s approval. Then, attending person needs to sign Application/Permission Form for Family Attendance.
    ※Please contact your nurse.

Day Leave & Furlough

  • Please kindly refer to the guidance of during hospitalization (orientation at your admission) at your ward.
    ※You will need to sign Application/Permission for Request Day Leave and Furlough.

Discharge

  • You will be notified your discharge date from your physician or nurse by the day before discharge. However, you may be arranged for your discharge on the very day. We ask for your understanding and cooperation.
  • The time of discharges is basically in the morning.
  • Please make sure check with your physician or nurse for your medications, discharge guidance and next appointment. If you have medications, it will be provided at your ward.
  • You will be notified from ward staff as soon as when your hospitalization expenses is ready, then please make your payment at the Inpatient Flow Management Center on the 1st floor for your payment during weekdays (weekdays: Monday to Friday 8:30 to 16:00, Saturday 8:30 to 12:00). During Saturday afternoon, Sunday and holiday, please make your payment at After-hour Outpatient counter on the 2nd floor. After paid your hospitalization expenses, you will be provided discharge certificate.
  • Please ask staff in advance if you need any documents prior to leaving hospital.
  • Please let our staff know if you have any request for consultation about payment of your medical expenses. The staff in charge will be responded.

Payment for Hospitalization

  • Please make your hospitalization fee payment at the Inpatient Flow Management Center on the 1st floor (for inpatient cashier counter) at the day of you discharge.
  • For estimated hospitalization fee, please ask ward nurse or staff at Medical Affairs (2nd floor).
  • We accept various credit card. (Please kindly refer to the guidance of during hospitalization (orientation at your admission) at your ward.
  • Please keep your receipt in a safe. You may be needed for various procedures at later date.
  • For the patient who continued hospitalization, we will deliver the bill to your room on the around 15th of next month after closed at the end of the month.
    Please make your payment at inpatient cashier counter on the 2nd floor. (at After-hour Outpatient)
  • We have a consultation counter at general reception regarding medical fee payment. (payment consultant staff at Medical Affairs)

No Alcohol & No Smoking

  • Our hospital is a non-smoking facility including parking lot.
  • In addition, alcohol drinking is prohibited during hospitalization.
    ※Please refrain from drinking non-alcohol drinks also.

Baggage & Electronics

  • Please make sure to write your name on your personal belongings. Bringing electronics to the hospital is not allowed. If you need to bring any electronics, please let nurses know.

Valuables

  • Many other people come and go to the hospital, therefore, it is an environment where unexpected accidents such as stolen or loss are likely to occur.
    Please do not bring a large amount of cash or valuable items to the hospital. If you need to bring those to the hospital, please manage those at your own risk, such as keeping those in the safety box.
  • Please check at your staying ward for using safety box. In the event of stolen or loss, our hospital will not bear any responsibility.
  • Our hospital also will not bear any responsibility of false teeth or hearing aid stolen or loss so that please strictly manage your personal belongings by family members.

Cell-phone

  • At our hospital, except in some areas, cell-phone can be used in the hospital. We kindly request your cooperation for your good manner and not disturb others when you use.
  • For privacy protection, it is prohibited to take photos or video recording in hospital (except authorized photo shoots only).
  • Cell-phone use limited area: Cell-phone can use only at family waiting room where the area of ICU, HCU and Operation Room (4th floor area).

Others

  • If there are any malicious disturbances to other patients’ treatment or not obeying appropriate hospital stay, you may be discharged by force. Moreover, please let staff know when you are subject to attack or see any disturbances or suspicious person.
